It takes an enormous amount of time and effort to secure the medication and ensure a continuous supply for our current patients. Because of the extraordinarily high demand, we are only able to accept a limited number of patients. This ensures that our current patients have full access to the medication they need to meet their health goals. Please submit your appointment request and questionnaire and we will notify you as soon as possible if there is an issue with your request.
A $50 nonrefundable deposit is required for all new patient appointments. You will receive an invoice via email after your appointment is scheduled. At the appointment, the $50 will be applied to the cost of your weight-loss program. If you cancel your appointment with less than 24 hours notice or "No show" for your appointment, the $50 will be forfeited.
There will be a $35 charge for all follow-up appointments. If you have questions or concerns about your medication, including questions about dosing, medication effects, side effects, adverse reactions, YOU MUST MAKE AN APPOINTMENT. This includes in-office appointments, Telemed appointments, and Phone Call appointments.
Please be on time for your appointments. If you arrive 15 minutes or later for your appointment time, your appointment may be canceled and you will be subject to a $25 fee.
If you are unable to make your appointment, kindly give us 24 hours notice or you will be subject to a $25 fee. If you no show more than once, you will not be allowed to make any future appointments.
