There are 4 steps to the Vampire Facial®

The primary goal in skin rejuvenation has long been to stimulate collagen production and restore lost collagen to the face. To achieve this, the skin has been chemically peeled, burned, cut, lasered, injected, and scraped, to name a few. Ouch! There is a better option!
Microneedling (or collagen induction therapy) is the process of using a stamp, roller or hand-held electronic device that utilizes tiny needles to make precise microscopic insults and injuries to the skin. Microneedling and PRP work synergistically together. In this case, it's like 1 + 1 = 10 because of the way these two treatments complement each other. PRP or platelet-rich plasma is an incredible substance derived from your own blood that contains a concentrate of over 1100 healing growth factors and other beneficial proteins. When applied to microneedled skin, the PRP is absorbed through the microchannels and the growth factors in PRP activate multipotent stem cells already in the skin (tricking them into thinking there’s been an injury and new tissue should generated). The platelets are like small containers of growth factors and cytokines that cause cell migration and recruit pluripotent stem cells to the area from the bone marrow, the liver, or the local tissue. HOW PRP IS MADE:
A small sample of blood is drawn and placed into a device called a centrifuge. Based on the most recent and up-to-date research, we have added LED red and infrared light as part of our treatment. LED light therapy is a form of Photobiomodulation (PBM Therapy) previously known as Low Level Laser Therapy (LLLT). The inclusion of light therapy TRULY sets us apart from the others. Vampire facials combine the powers of microneedling and PRP," says Melissa Doft, MD, a board-certified plastic surgeon in New York City. "During a traditional microneedling treatment, needles create tiny insults in the skin, stimulating the skin to repair itself with new collagen and elastin.” But in a vampire facial, this is coupled with platelet-rich plasma, which “contains growth factors to help stimulate repair.”
The ideal candidate will be anyone who is interested in minimally invasive procedures that are more natural yet very effective at reducing the visible signs of aging and skin laxity. In other words, patients who desire measurable clinical results from treatments with little to no recovery. The patient must also have realistic expectations and realize that it may take several months to see the full effects of treatments.
Microneedling has many indications including: Mild to moderate acne scarring, fine lines and wrinkles, uneven tone and texture, crows feet and lip lines, loose and lax skin, pore size, sun damaged skin, superficial pigmentation, hypopigmentation, surgical scars, and stretch marks.

Precautions should be taken in the following patients: hypothyroidism (dry skin, pigmentation issues), hay fever/asthma/ food allergies and sensitivities (histamine response, hyperpigmentation), anemia (impaired healing), Polycystic Ovarian Syndrome (PCOS) (acne, pigmentation issues), Inflammatory bowel disease (altered immune status), diabetes (impaired healing, risk on infection and and glycation), psoriasis (Koebner phenomenon where lesions occur at site of micro-injury), and eczema/dermatitis (disrupted barrier function, increased dryness).
Lesser results may occur in smokers, excessive alcohol use, menopausal patients, low fat diet, excessive caffeine intake, and excessive UV exposure.
PRP or platelet-rich plasma is a substance derived from a whole blood that contains large amounts of proteins and growth factors which contribute to a biological cascade of activities that stimulate new blood vessel formation, tissue repair, cell multiplication, and overall healing. Prepared PRP contains more than 1100 different proteins used in regeneration and remodeling of tissue and new blood vessel growth. Studies have shown a direct correlation between increase platelet concentrations, and the proliferation of human mesenchymal, stem cells, fibroblast growth, and the synthesis of type I collagen, and therefore amplifying the whole healing process.
A normal body produces 1 billion new platelets a day. The 2 tubes of blood drawn for PRP is approximately 1-2 tablespoons of blood, the same or even less blood drawn for routine lab work. For optimal results, it is best to ensure you drink plenty of fluids and are well-hydrated prior to your treatment.
Contraindications for PRP treatment include coagulopathy (blood clots), known platelet dysfunction, hemodynamic instability (low BP, etc), severe hypovolemia, sepsis, unstable angina, HIV, anticoagulant therapy, low platelet count (<1000) or low hemoglobin count (<10.9), non-dysfunctional platelets, or clotting disorders, NSAIDS/Aspirin products in the past 7 days, anticoagulant use in the past 5 days, recent corticosteroids (within 6 weeks), active signs of systemic infection, active cancer, rash or unclear skin changes at the treatment area, pregnancy, breastfeeding, hypertrophic diseases (scars, keloids) and severe metabolic and/or systemic disorders (uncontrolled diabetes or severe asthma).
Red light therapy, also known as photobiomodulation or low-level laser therapy (LLLT), is generally considered safe. However, there are a few cases where caution is recommended or the therapy should be avoided altogether:

You should start seeing visible results in a few days after the PRP settles in. Within a few weeks to a few months, skin will look visibly plumper, fine lines and dark circles will be diminished. Overall, you will have a brighter, tighter, more even, and youthful glow.
The side effects include mild discomfort, redness, swelling, and possible infection. These are temporary. If redness persists or drainage starts, please inform the office immediately.
In-office microneedling (with or without the addition of PRP injections) must be administered no sooner than every four to six weeks. This is due to the natural healing process that must occur post-needling. Microneedling sooner than 28 days after treatment can interrupt the natural cycle of skin healing and trigger inflammation which can negatively affect outcomes.
In certain circumstances, the answer is yes. For wrinkles that appear the first few days after microneedling, inadequate hydration status is the primary cause. Inadequate water intake, especially along with exercise or sauna use, can can leave you poorly hydrated. Patients who are taking any type of diuretic or blood pressure medication are also at risk for poor hydration. Patients who are peri-menopausal and/or post-menopausal often have issues with dry skin due to hormonal influences and microneedling will temporarily exacerbate this condition.
Wrinkles that worsen after several weeks or months often have glycation as the cause. Patients who have elevated blood sugar or diabetes are at risk for this complication. Glycation is the abnormal cross-linking of sugars and proteins under the skin. The abnormal molecules that form are known as AGEs or Advanced Glvcation End Products. The body then creates antibodies in response to this foreign material which cause accelerated aging of the skin, including wrinkles and loss of elasticity. Patients with diabetes or elevated blood sugars have a much higher risk of glycation.
A lot of confusion exists when it comes to these terms. The FDA does not regulate any procedures. Here is a link to the official FDA page that discusses this in detail. Here is another article on what the term "FDA Approved" really means. Prior to 2017, there were no electronic medical needling devices approved by the FDA, yet use was widespread. These devices were initially incorrectly registered with the FDA under the class "micro dermabrasion" devices, which are generally exempt from certain requirements. Microdermabrasion removes layers of skin via sheer force. Micro needling devices penetrate the skin, and are now appropriately reclassified as medical devices. Here is a link to the FDA consumer page on microneedling. The FDA does not regulate the practice of medicine, but rather controls which medications/products/devices are available commercially.
PRP is exempt from FDA regulation and regulated solely under section 361 of the PHS Act. From the FDA’s guidance: Regulatory Considerations for Human Cells, Tissues, and Cellular and Tissue-Based Products: Minimal Manipulation and Homologous Use; then

“This guidance also does not apply to products that fall outside the definition of HCT/P in 21 CFR 1271.3(d). For example, platelet rich plasma (PRP, blood taken from an individual and given back to the same individual as platelet rich plasma) is not an HCT/P under 21 CFR Part 1271 because it is a blood product. Accordingly, FDA does not apply the criteria in 21 CFR 1271.10(a) to PRP, and PRP is outside the scope of this guidance.”
The FDA does NOT regulate the procedures performed, but they DO regulate the devices used to perform microneedling and prepare PRP. Make sure whoever you choose for these types of procedures are utilizing FDA Registered equipment. Anyone can buy a cheap centrifuge and cheap blood tubes off of Amazon, but that does not mean they are meant to prepare PRP.
